Follow-ups to #432 (part of #381): drop the stale coverage pragma on
make_file_change and document it, replace the tuple dict in modify_files
with two dicts, restore already-written files when a later write raises
OSError, and add tests for ignored files, write failure, make_file_change
writes and do_bump leaving config untouched.
