Critical Fix: Inflated Token Savings

• ctx_search inflated savings — Token counting now only considers files with actual matches, not all scanned files. A single search on a large project could previously report 119M+ "saved" tokens.
• ctx_tree inflated savings — Raw tree comparison now respects .gitignore, preventing artificial 100% compression rates.
