- New midroll blocking algorithm for the "Proxy ad requests only" mode (Fixes #196)

- Simplified passport options: now using a slider to select the passport type instead of the confusing "Use my passport" and "Laissez-passer" checkboxes. (Note: While the slider options are identical in Firefox and Chrome, their behavior is slightly different.)
- Added a proxy usage section: tells you if your proxy usage is low, medium or high
- Moved "Backup", "Restore" and "Reset" buttons to the top of the page

- Added "Proxy ad requests only" mode: optimizes proxy usage by only proxying requests that contain ads. Of course, this uses the new midroll blocking algorithm
- Added "Anonymous mode": watch streams as if you were logged out
- Fixed bug on Kiwi browser where extension would sometimes report PAC script as not set
