- Mark empty failed live sessions explicitly (#509 by @kkarimi) — Live sessions that end without a transcript are now classified as either "No audio captured" or "Transcription failed" instead of showing a generic "No transcript" state. The post-session banner uses a warning style for failed sessions, Notes/history views surface the specific reason and offer a direct "Re-transcribe" button when retained audio exists, and the failure marker clears automatically when a transcript is later recovered.
