Phase 302 R4 — navbar Liv tool-approval fix
When a navbar Liv prompt made the agent ask for a tool permission (e.g. reading an attached photo), the inline approval was killed by the 45s "no response" watchdog before the operator could answer — which aborted the stream and left the conversation locked, so follow-up messages returned HTTP 409 and the bar said "Liv did not respond in time."
- The silence watchdog now stops when an approval surfaces (the operator can take their time) and re-arms when they confirm.
- A 409 (busy conversation) now shows an actionable message + Open in Liv instead of a cryptic error.
