Fixes

• Always shows a Claude re-authenticate action in Settings when the Claude CLI is installed.
• Shows the Claude re-authenticate action on the usage panel whenever Claude usage fetches are unhealthy, including auth failures and rate-limit failures.
• Opens claude auth login in a visible Terminal session instead of a detached background process, so OAuth prompts and CLI interaction are not hidden.
• Waits for Claude keychain credentials to change before hitting the usage API again, reducing self-inflicted rate limited errors.
• Preserves last good usage values while clearly marking stale provider data.
