Versioned database migration system
Replaces the previous approach of re-running every schema check on every startup. Yarnl now tracks which migrations have been applied in a schema_migrations table and only runs what is new. On a fresh install all migrations run once; on every restart after that, none do. When future versions add schema changes, only the new steps run.
This also resolves a long-standing issue for users on shared or external PostgreSQL instances who were seeing "permission denied for schema public" on startup. This was caused by a PostgreSQL 15 behavior change that revoked the default CREATE privilege on the public schema. The fix is not a superuser requirement — it is a one-line grant that any database owner can run:
GRANT CREATE ON SCHEMA public TO yarnl;
