Status-aware admin notifications
The notification and PENDING digest treated every request as a fresh approval, and approve replies never echoed what changed.
- REJECTED requests now say "Was declined earlier — APPROVE to re-submit" instead of the standard approve/decline footer.
- PENDING digest lists actionable REJECTED requests in their own "Declined" section, so
approve <ID>targets are visible before they're typed. - Approve success replies echo the previous state: "approved and submitted (was REJECTED)".
