Session-scoped Main and Worker models

ACP clients can now choose the Main model, the Worker model and both efforts per session, without one session changing another.

session/new, session/load and session/resume accept _meta.modelId, reasoningEffort, reasoningEffortAuto, workerModelId and workerEffort, applied to that session only before its first turn. An explicit level wins over auto.
New x.ai/session/worker_model/set ({sessionId, modelId, effort}) sets the Worker for a running session: "" turns the Worker off, null returns to [models] worker from config. It answers with the effective Worker and whether it comes from the session or the config.
session/set_model and session/set_config_option no longer change the process-wide model, effort or auto mode, so later sessions without _meta start from config. The TUI keeps its own defaults.
The session/set_model response now also returns canonicalModelId, reasoningEffort and reasoningEffortAuto.
initialize advertises sessionMainModel and sessionWorkerModel under x.ai/capabilities.
