Prerelease. Adds the zombie-joystick recovery from the Bergamo community build (#67, credit @bg2-design): if a controller disconnect/reconnect leaves SDL with a dead handle while the console still sees your inputs, the handle is reopened within a quarter second, no restart needed. Also carries everything from pre5 (server-side input wedge detection and revive for #61).
