Bug Fix
Fix duplicate header spans causing column misalignment — Some PDFs render table headers twice at the same position (duplicate content stream operators). collectHeaders now deduplicates spans sharing the same X coordinate and text, preventing FindTableAcrossPages from creating misaligned column indices across pages.
