Fixes from braklo's testing of 0.14.0. The most important is doc_open
on a file that was already open, which could leave it open with no lock
file (GitHub #40).

Fixed

• doc_open on a file that was already open loaded it a second time
  (GitHub #40). The result was two documents over one file, two doc_ids
  both reported active, and closing either one removed the lock file while
  the other stayed open, so another application could open the file for
  writing without warning. doc_open now switches to the open document
  and returns its doc_id with already_open: true. doc_list_open marks
  the active document by identity, not by URL and title
• /health could report a stale active document after doc_open
  (GitHub #41). It answers from a snapshot kept by document events, which
  a document loaded over MCP did not always update. The snapshot is now
  refreshed at the end of every tool call, on the main thread
• A chart name freed by a delete was never used again (GitHub #32,
  follow-up): new charts now get the lowest free Chart_N
• Some Calc errors carried their reason in error instead of
  message (GitHub #33, follow-up). Every error result now has
  message; error stays for clients that read it
