PersistenceCore v9.2.1 Patch Upgrade

Upgrade v9.2.0 as intended, but
Includes fork logic to fix chain halt and repair the state. Tested via a unit test and E2E ictest.

Main changes

• github.com/persistenceOne/cosmos-sdk upgraded to v0.47.3-lsm5, see main PR that amends faulty logic and prevents halting.
• Includes upgrade handler to migrate the state
• Includes testing suite covering the migration and binary upgrade
