Fixes

• Page titles in browser tabs (#15, thanks @tilwegener): tabs no longer all say
  frontend. Each page now sets a title such as Certificates | PKI Manager or
  SSH Host Details | PKI Manager, and it updates when you navigate or go back. Unknown routes
  show PKI Manager. Route parameters and query values are never included in the title.
• Issuing CA link in the certificate list (#16, thanks @tilwegener): clicking the issuing CA
  opened /cas/undefined ("Certificate Authority Not Found"), because the list API returns
  caId, not issuingCA. The link now opens the right CA.
• CA page layout and dark bulk dialogs (#17, thanks @tilwegener): the Back, Revoke and
  Delete buttons now sit above a separate, labelled export panel, which also works on small
  screens. That panel holds the format, the generated export password and Download, with
  Copy Link as a secondary button. The bulk Renew/Revoke/Delete confirmation dialogs follow the
  theme, so the title and Cancel button are readable in dark mode. Renew is shown in the
  primary colour, while revoke/delete stay red. The dialogs are also labelled for screen readers.

Known issues

• Unchanged from v3.14.1: export passwords still travel in request URLs, and the CA page's
  download URLs and Copy Link still include them (TASK-237).
