v11.9.1 — Make Define honor --timeout, finish when a provider seat fails, and skip an out-of-quota Antigravity seat

Fixed

• --timeout now reaches every synchronous provider call. Grasp passed a fixed
  300-second budget to each seat that the flag could not raise, so a retry with
  a larger --timeout failed the same way. OCTOPUS_AGENT_TIMEOUT still takes
  precedence, calls that run deliberately unbounded stay unbounded, and
  --timeout 0 does not remove their bounds. Council seats keep the budget from
  --seat-timeout and OCTOPUS_COUNCIL_TIMEOUT_<PROVIDER>.
• A failed or timed-out constraints seat no longer ends a standalone grasp run
  before consensus and discards the perspectives already gathered. When every
  seat fails, grasp now reports that and writes no consensus file.
• Grasp skips an Antigravity seat already marked quota-dead instead of
  dispatching it again and waiting through its quota retries. Consensus falls
  back to Claude when Antigravity is unavailable or returns nothing, and the
  consensus file records which provider synthesized it.
• The timeout message recommends OCTOPUS_AGENT_TIMEOUT when that variable is
  set, because it overrides --timeout.
