• Learned file hints and provider history stay within the current project or
  Git worktree. Hints record and suggest only existing relative paths inside
  that root. Older patterns without a root and unscoped provider histories
  remain on disk but are ignored. Provider history now uses
  .octo/providers/<project-key>/ under the Octopus workspace.

• Provider answers stay readable when stderr contains a large transcript.
  With a usable stdout answer and stderr over 16 KB, the full transcript goes
  to a private .stderr.log sidecar. The result keeps at most 16 KB from its
  last 40 lines, preserving UTF-8 and the untrusted-content wrapper. Runs
  without an answer, unsafe sidecar paths, and transcripts over the private
  writer's 1 MiB limit keep stderr inline. Result expiry removes the matching
  transcript while preserving transcripts for retained reports.

• Smoke-test failures no longer mark a healthy provider quota-dead for later
  runs unless they are explicit authentication or rate-limit rejections
  observed outside coding sandboxes. Other failures exclude the provider only
  for the current run. Unknown failures prevent caching a successful overall
  check, while timeouts retain the existing slow-provider pass-cache policy.
  Preflight distinguishes cached quota/auth failures, with their individual
  timestamps, from current-run failures. Sandbox denials identify the check's
  restriction and advise retrying outside the sandbox.
