v2.0.1 - Fix setup crash (frozen middleware list)

Hotfix for the setup crash introduced in 2.0.0.

Fixed

• Setup failed with "Unknown error occurred" on every new install or re-auth, with RuntimeError: Cannot modify frozen list in the log (#39, #40, #41). Home Assistant freezes its HTTP middleware list at startup, and 2.0.0 tried to add a middleware during the config flow, long after that. The security-filter exemption needed for Microsoft's silent SSO redirects is now applied without touching the middleware list. It stays scoped to the temporary auth proxy path; every other request keeps Home Assistant's security filter unchanged.

The integration and the auth add-on are both at 2.0.1. Nothing else changes from 2.0.0; see the 2.0.0 release notes for the native authentication flow and the entity-id breaking change.
