Fixed:
- Disposing or reconnecting a WebSocket connection could freeze single-threaded hosts such as Blazor WebAssembly (#1269)
- The ping timer stopped after a PING write timeout, so a dead connection could go undetected. A write timeout now counts as a missed pong (#1266)
