- fix: doctor reads codex plugin list --json whole, no 1 MiB cut (#1322)
- fix: Codex Stop hook decodes and gets the empty reply; doctor D5 sends a Stop probe (#1323)
- fix: Read size deny skips images and PDFs the host shows as blocks (#1321, #1324)
- fix: check every agent's hook replies against its host's own reply schema, in unit tests and doctor D5
