Hotfix for v1.0.58 — missing PreToolUse hook in Codex config and stop hook in Cursor config.

Fixes

• Codex CLI: Added PreToolUse hook to configs/codex/hooks.json and README install instructions. PreToolUse enforces sandbox routing (was missing in v1.0.58).
• Codex CLI: Removed all stale "no hook support" references from README continuity table and session descriptions.
• Cursor: Added stop hook entry to configs/cursor/hooks.json.
