Fixes (#156 — Windows/nvm4w)

• Replace deprecated npm bin -g with npm config get prefix (removed in npm v9+)
• Detect ALL shim locations via where context-mode.cmd — creates directory junctions at every stale shim location (e.g., C:\nvm4w\nodejs\) so they can resolve the package
• Auto-fix stale shims that reference old build\cli.js → rewrites them to use cli.bundle.mjs
