Changes

• Proxy manifest requests when "Low Latency Player" is disabled
• Moved settings cell to account page

I've started hosting my own proxy servers, if needed you can use either of these two:

• proxy.level3tjg.me:6375 (default)
• https://luminous.level3tjg.me

Also consider supporting TTV LOL PRO devs to get access to their restricted proxy servers.

Note that inputing an address that starts with "http" will be parsed as a luminous / ttv.lol v1 proxy server and anything else will be parsed as a standard http(s) proxy server.
