• SignalDetail.monitoring_state gains a failed variant, returned when the most recent evaluation generation or realignment workflow for the signal ended in failure. It carries phase ("generate" or "realign"), an optional evaluation_id (present only for realign), and a nullable reason with the resolved failure message. A later successful workflow supersedes an older failure, so failed only reflects the latest run.
