Fixed: the extension failed to connect on some SSMS 22 builds

On affected SSMS versions every feature failed at once, with:

System.TypeInitializationException: The type initializer for
'Microsoft.Data.SqlClient.SqlConnection' threw an exception.
---> System.IO.FileNotFoundException: Could not load file or assembly
'System.Diagnostics.DiagnosticSource, Version=10.0.0.3, ...'

The extension shipped Microsoft.Data.SqlClient but not the assemblies SqlClient
itself binds to. Those loads only ever succeeded because the host SSMS build
happened to carry a binding redirect that reached far enough, and that ceiling is
lower on older SSMS builds — so the extension worked on one SSMS 22 and was
entirely dead on another. Reported on 22.5.2; 22.10 was unaffected.

The container now carries SqlClient's full runtime dependency set at the exact
versions it binds to, so connecting no longer depends on what the host shell
happens to redirect. Where SSMS does provide a redirect, its own assemblies are
still used — nothing about a working install changes.

If you hit this, install this build and restart SSMS.

Also in this build

  • Formatter: INSERT column layout options work again. The underlying T-SQL
generator changed how it emits an INSERT column list, which left "columns per
line" and both open-bracket options doing nothing. The formatter now owns that
    layout outright.
  • Formatter: CROSS APPLY and OUTER APPLY are lined up with FROM, the same way a
JOIN already was, instead of sitting at whatever column the generator chose.
A stacked derived table under an APPLY follows it.
  • Updated Microsoft.Data.SqlClient, SQL Server Management Objects and the T-SQL
    parser to current versions.
