Fixed

• XHTTP URL missing sni and fp parameters — Shadowrocket (and potentially other clients) failed to connect via XHTTP because the URL lacked TLS parameters. Reality URLs had them; XHTTP URLs did not. Fixed in all 4 URL generation locations.
• Rich markup eating [y/N] prompt hints — the relay offer prompt showed no hint about how to decline because Rich parsed [y/N] as a markup tag. All y/n prompts now display correctly.

• Replaced qrencode binary with Python segno package — QR codes are now generated in pure Python. No more broken QR codes when system qrencode is not installed. Removes the qrencode apt dependency from server provisioning too.
