Fixed

• Move/Swap hotkeys use wrong screen's layout: When a window was moved between monitors externally (KDE's Move-to-Screen shortcut, manual drag racing with outputChanged), keyboard navigation hotkeys used the stored screen assignment instead of the window's actual screen, snapping the window back to the old monitor or computing zone geometry from the wrong layout. Now always uses the effect-provided screen (EffectWindow::screen()) and detects stale assignments: if the stored screen differs from the actual screen, treats the window as unsnapped and navigates from scratch on the correct screen's layout.
