Blu-ray disc menus

Fixed: some BD-J discs opened to a black screen instead of their menu.
Menu graphics were being drawn before their images had finished decoding;
the draw was then discarded with nothing left to repaint it, so the menu
ended up composed of nothing. Drawing now waits for the decode. -
Fixed: playback could stall with no picture at a title change. A decoder
held over a clip change could outlive a failed stream open and be reused by
the next title, still carrying the previous clip's timestamps. The player
then started its clock a long way from the audio and never presented a
frame.
