  headers now raise a clear error pointing to proxy/CDN misconfiguration instead
  of silently producing incorrect behaviour.
• e3b3134: Fix infinite loop when CDN serves stale cached responses to a client with a
  valid local handle. The client now always retries with a cache-buster parameter
  to bypass stale CDN caches.
• 9335ae6: Add fast-loop detection to prevent the client from hammering the server when
  responses don't advance the offset. Matches the TypeScript client's behaviour
  of detecting, backing off, and eventually erroring on stuck retry loops.
