Add exposed option to modular-mcp feature (#545) - Introduces an optional exposed boolean field to MCP server configurations in .rulesync/mcp.json. When modular-mcp is enabled and a server has exposed: true, that server will be output directly to .mcp.json and not included in modular-mcp.json. This allows frequently-used MCP servers to be directly accessible while less-used servers remain compressed behind modular-mcp, optimizing both performance and context usage.

Remove MCP configuration files from git tracking (#543) - Removed MCP configuration files that were previously tracked in git to maintain a cleaner repository state.
Track Claude Code configuration files in git (#542) - Added tracking for Claude Code configuration files to improve project setup consistency.
