PR #2292 by pcbeingused333 and ksylvan: fix(openai): return the error of a failed Responses API stream
- Fixed the OpenAI integration so that errors from a failed Responses API stream are properly returned instead of being silently ignored.
- Simplified error handling in the OpenAI response stream by returning stream errors before processing response failure events.
- Formatted response failures inline with the associated message and error code for clearer diagnostics.
- Removed the standalone helper used to format response failure errors, reducing redundant code.
- Ensured trailing content is emitted only after the stream is confirmed successful.
