PR #2293 by pcbeingused333 and ksylvan: fix(codex): return the error of a failed Responses stream and report its usage
- Fixed the Codex plugin so that errors from a failed Responses stream are returned to the caller and the associated usage is reported.
- Returned response failure errors immediately during streaming instead of deferring their handling.
- Included error codes in failure messages when they are available.
- Removed the redundant error helper and its fallback message.
- Added test coverage in
internal/plugins/ai/codex/codex_test.go.
