- Tool results are marked failed far more reliably — A tool run that died without an explicit error payload used to render as a neutral notice, hiding real failures behind a calm card. The browser now recognizes the common shapes of a dead run and marks it FAILED.
- Tool results ending in an exit-code or signal marker are failed; quoted mentions of the same markers still read as OK.
- Persistent-shell death markers fail the run, while a clean shell exit stays a notice.
- Killed or failed background jobs are failed via their terminal
job_outputstatus line.
Also in this patch: trend-chart polish — the card reserves its scrollbar gutter so its height stays invariant, and unified detail-num widths keep delta rows from jumping on hover. （#19）
