• 专注模式的等待回复卡片中，计划审查（exit_plan_mode）的计划正文现在通过官方 Markdown 渲染器呈现——标题、列表、代码块不再以 #、1. 等原始语法漏出；卡片标题同步显示为「计划待审 / Plan review」（仅呈现层变化，答案编码协议不变）。
  In focus mode's waiting-reply card, the plan-review body (from exit_plan_mode) is now rendered through the official Markdown renderer — headings, lists and code fences no longer leak as literal # / 1. syntax; the card title reads "Plan review" for plan-review intents (presentation only, answer encoding unchanged).
