Fixes

Rows could stay permanently blank on load when the first hass update arrived after the initial config render — the likely cause of long-standing intermittent blank rows (#389).
Rows with % entities clipped on narrow/mobile screens instead of wrapping — regression from 4.6.0's switch to HA-native formatting (#403).
unit: overrides (including unit: false) were ignored on attribute displays — same regression family (#408).
format: time/datetime now works on the last-changed/last-updated attributes (#221, #305).
default: headers resolve via the friendly name — no explicit name: needed (#302).
confirmation: now gates toggle: true toggles (#265).
name: false values align with headered siblings (#281).
