Bug fixes

• cf config init-env --force keeps a custom CF_WEB_USERNAME. It used to be replaced by the commented # CF_WEB_USERNAME=admin template, silently changing the web UI login. An existing CF_WEB_USERNAME and CF_WEB_PASSWORD are now written back exactly as they appear in the old .env (quotes, $, escapes, export, comments and multiline values included). When a password is generated next to a kept username, the output says Web UI login: CF_WEB_USERNAME from .env / <password>.
• No crash on a malformed .env. A line such as a quoted key ('CF_WEB_PASSWORD'=x) made init-env --force fail with an IndexError; such a definition now counts as unset and a new password is generated.
• Docs: init-env rewrites the whole .env and keeps only CF_WEB_PASSWORD and CF_WEB_USERNAME, as written.
