notify hook: improved back-navigation in Kitty + tmux — recovers tmux
client metadata so detection works when TERM_PROGRAM=tmux masks the
terminal, and the click-to-navigate command now chains open → kitty
focus-tab → tmux switch-client → select-window → select-pane.
notify hook: less noise — skips idle_prompt when the tmux pane is
already focused (permission prompts still always fire), recovers the real
TERM_PROGRAM from the tmux server env so iTerm/WezTerm under tmux activate
the right app instead of Terminal.app, and drops the last-commit-subject
substitution that cluttered idle messages.
