Toast gradient tones
Xbox-style unlock toast now uses 3-stop vertical gradients per rarity (lighter → base → darker) instead of flat fills. Mythic, Legendary and Epic especially read with proper depth instead of looking neon and flat. Rarity palette also realigned to the canonical rainbow used elsewhere in the plugin:

README banner replaced with a looping animated SVG that literally reproduces the Xbox achievement popup - circle expands into a banner, trophy rotates inside, rarity cycles through all six tiers on a 30-second master loop so visitors see every gradient in action.
