Urgent: fixes playback crashing about 20 seconds in on real Apple TVs.

Fixed

• App crashed around 21 seconds into heavy 4K remuxes. The deep disk backed buffer introduced in 0.2.11 behaved on the simulator but filled toward a hard memory ceiling on real hardware, killing the app at an almost constant 21 seconds into high bitrate Dolby Vision remuxes. Playback caching is back on the configuration that ran clean for weeks (half a gigabyte of forward buffer). A deeper buffer will return only after it survives soak testing on a real Apple TV.

Everything from 0.2.12 (the vanishing tab bar root cause fix, instant player shutdown, the Watch button keeping focus) is included.
