Fix initial sync failing with "Cannot insert document ... from sync because it already exists" when a document is inserted and then updated while the initial fetch is running. The buffered insert event is now applied over the row that the initial read already returned. (#2336)

Remove a field from a synced row when an RxDB document update removes it. RxDB change events carry the whole document, so the adapter now replaces the row instead of merging the update into it. (#2336)
