New feature: Add, rename, and delete items in PDF outlines (a.k.a. table of contents / bookmarks)
- Added a new command "Add to outline"
- Added new items "Add top-level item" / "Add subitem" / "Rename" / "Delete" to the right-click menu in the PDF outline viewer.
