v1.6.5 — Auto-upgrade on every session start

Every active user will now automatically get the latest version.

SessionStart hook now backgrounds uv pip install --upgrade cozempic on every session start. Non-blocking — runs in background while guard starts immediately.

npm install now always upgrades (was: skip if already installed) and tries uv first.

How it reaches old users:

• Users who run cozempic init get the new hook → auto-upgrade on next session
• Users who do npm update -g cozempic get the new install.js → forces upgrade + re-wires hooks
• The 1.6.4 auto-updater (uv/pip/pipx fallback) bootstraps to 1.6.5, which then keeps them current via the hook
