Fixed
- Task detail: the Definition/Plan prompt now refreshes while planning and Plan Review are running, so you are not reading a stale plan.
- Engine: stale-active-branch reclaim no longer floods the log for done-task squash leftovers, and those completion branches are deleted once the task finishes.
- Engine: Missions lifecycle hooks fire correctly, task updates only emit real lane moves instead of phantom dependency re-spec events, and schema/ledger bookkeeping tracks the current migrations and worktree capacity readers.
