Fixes

• Windows: fcntl module not found — deferred Unix-only fcntl import in scsi.py, instance lock uses msvcrt.locking on Windows
• Windows: python-multipart missing — moved from dev to main dependencies (FastAPI runtime requirement for form/file uploads)
• macOS: missing app.icns — CI now generates .icns from trcc.png via iconutil at build time
