Fixed

• Metadata refresh now matches items that have no ID in their path, via the title/year search (previously the lookup returned early when there was no IMDb/TVDB/TMDB id, so id-less items never got searched).
• Manual matching now sticks. A manual (poster) match always marks the item tmdb-manual — even when TMDB has no original_language for the pick — and it now also matches a loose single file (the grid passes the file's own path, whose <file>/% prefix matched nothing), so re-matching no longer leaves items on heuristic.
