Fixes

• Prevent stale configuration or unrelated directories from falsely detecting VS Code Copilot.
• Stabilize verification when GENTLE_AI_CHANNEL is inherited from the environment.
• Replace the self-timing-out Pi CodeGraph probe with bounded direct MCP capability verification.
• Preserve pending configuration when adapter health cannot be verified safely, while keeping concrete protocol and process failures fatal with rollback.
