Brings Codex's skill registry up to parity with the other agents.

What changed

• Skill registry parity for Codex — the Codex SDD orchestrator now resolves and injects skills the same way the other agents do, so delegated phases load the right project standards.
• Permissions & SDD injection updated to support the registry wiring, with matching cleanup on uninstall.
• Docs refreshed — README.md, docs/usage.md, docs/intended-usage.md, and docs/skill-registry.md updated to reflect the parity.
