Recording Architecture Decisions

• Added recording-architecture-decisions as the dedicated ADR lifecycle
  skill for direct ADR, architecture decision record, decision log, and
  baseline sync closure requests.
• The new skill runs the ADR creation gate, chooses create / amend /
  supersede / skip, selects the owner surface, and closes Baseline Sync before
  any ADR or baseline writeback.
• Kept the boundary explicit: ADR handling remains advisory method-pack
  discipline and does not become completion authority, an authoritative
  GateDecision, or a PolicySnapshot.

Baseline Sync Closure

• Updated ADR Auto Backfill guidance so create, amend, supersede, needed, or
  unknown baseline sync outcomes route through recording-architecture-decisions.
• Extended verification-before-completion and requesting-code-review so
  completion and review flows flag missing ADR lifecycle handoff when durable
  architecture decisions or baseline sync closure are in scope.
• Documented the compact output contract in the workflow quality baseline:
  Decision Candidate, ADR Gate, ADR Action, Owner Surface, Baseline Sync, and
  Boundary.

Trigger And Release Coverage

• Added recording-architecture-decisions to doctor key skill detection and
  skill-triggering / explicit-skill request coverage.
• Expanded workflow quality fixtures and checks with direct ADR lifecycle and
  ADR-skip samples, including no-forced-ADR and no-forced-baseline-writeback
  guardrails.
• Bumped public package and host distribution manifests to 1.5.3.
