Completion-time ADR Backfill Check — extended
verification-before-completion with an advisory ADR Backfill Check that
records trigger status, suggested action, evidence source, baseline sync,
skip reason, and method-pack boundary.
Workflow quality guardrails — expanded the workflow quality baseline and
fixture coverage so ADR backfill behavior is locked as a representative
completion signal rather than a runtime authority decision.
Boundary preservation — kept ADR backfill as method-pack signal and
review discipline only; this release does not add authoritative
GateDecision, PolicySnapshot, evidence sufficiency, or
completion authority.

Aligned ADR signal handling across brainstorming, writing-plans,
long-task-continuation, requesting-code-review, and
verification-before-completion.
Preserved ADR signals, source references, alternatives, compatibility
questions, baseline-sync questions, work records, proof bundles, drift
checks, and evidence references across workflow handoffs.
Clarified that long-task records and verification evidence are preferred
sources for ADR Auto Backfill review.

Demoted requesting-code-review from the default generic completion path so
normal completion claims route to verification-before-completion.
Retained code review as an explicit or high-risk independent escalation path
with representative trigger-health and workflow-quality coverage.
Made review baseline-aware by requiring reviewers to distinguish baseline
defects from architecture drift and to check ADR Auto Backfill / baseline
sync findings.

Marked agents/code-reviewer.md as a host compatibility projection rather
than a second canonical owner.
Kept skills/requesting-code-review/code-reviewer.md as the canonical
reviewer template while mirroring the key baseline-aware and ADR review
semantics into the compatibility projection.
