Added a fix for 100% volume spikes

There is a fault between some audio device drivers and some versions of Windows. This fault causes the audio devices to hit 100% volume when either Voicemeeter's audio engine is reset or the default audio device changes. This release adds a toggle, "Prevent 100% Volume Spikes", which attempts to work around the issue. With this option enabled, you can still obtain 100% volume if you desire, but you will need to use the volume slider or volume up keys to do so.
