fix(a11y): F-V54-A — #/cv single <h1>.

🐛 Fixes

• The CV markdown's own # Name rendered as a second top-level <h1> next to the page-title <h1>CV</h1> (regression run F-V54-A; WCAG 1.3.1 Info & Relationships / 2.4.6 Headings). cv.js now feeds every preview-injection point (initial render, file-import refresh, live editor sync) through a scoped cvMd() that shifts headings down one level (h1→h2 … h5→h6, h6→role="heading" aria-level="7"), so the page keeps exactly one <h1>. Scoped to cv.js on purpose — UI.md is shared by help/reports/deep/evaluate, which manage headings their own way (help strips article h1s + builds its TOC from h2). Verified live: #/cv → 1 page <h1> ("CV"), the user's # Name is now <h2>, 0 console errors.

🧪 Tests

• test: tests/cv-single-h1.test.mjs — 4 cases (cvMd shift chain, every preview site uses cvMd not raw UI.md, single page-title h1, the transform re-derived + proven to map every level down). 717 → 721.
